Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 allowing for multiple and diverse meta-epidemiological research studies to examine the effects of treatment across trials with different levels of pragmatism as well as other design features.
Background
Pragmatic trials are becoming more widely acknowledged as providing evidence from the real world to support clinical decision-making. However, the use of the term "pragmatic" is not consistent and its definition and evaluation requires clarification. Pragmatic trials are intended to guide clinical practices and policy decisions, not to confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should try to be as similar to actual clinical practice as possible, such as its selection of participants, setting up and design, the delivery and implementation of the intervention, as well as the determination and analysis of outcomes as well as primary analysis. The difference in the main analysis domain could be explained by the fact that the majority of pragmatic trials analyse their data in the intention to treat way, whereas some explanatory trials do not. The overall score for pragmatic systematic reviews was lower when the domains of management, flexible delivery and following-up were combined.
It is important to remember that a pragmatic trial doesn't necessarily mean a low-quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but this is neither specific nor sensitive) that use the term "pragmatic" in their abstracts or titles. These terms may signal an increased appreciation of pragmatism in abstracts and titles, however it's unclear whether this is evident in the content.
Conclusions
In recent times, pragmatic trials are gaining popularity in research as the importance of real-world evidence is increasingly recognized.
